Percutaneous radiofrequency ablation (RFA) of S5 lesion, laparoscopic liver segmentectomy S5,6

Ефанов М.Г., Куликова Н.Д., Мелехина О.В.(РЧА)
Patient Z. 56 years old.
Diagnosis: Hepatocellular carcinoma S5.6 cT2NxM0
Associated: Chronic viral hepatitis C, minimal activity. Child-Pugh grade A cirrhosis (5 points)
Tumor markers
AFP 152 IU / ml
CA 19.9 27.9 U / ml
CEA 3.6 ng / ml
MRI 10/28/20:
in S6, a focal rounded formation with clear uneven contours, heterogeneous structure, dimensions 45x38x41 mm of hyperintense MR signal in the T2-WI, DWI mode, hypointensive on ADC, T1-WI is determined. Along the lower contour it is closely adjacent to the right kidney, without signs of invasion. With contrast enhancement, it actively accumulates the contrast agent in the arterial phase, followed by its “washing out” in the venous and delayed (hepatospecific) scanning phases at 10, 20 and 30 minutes. One gets the impression that there is a capsule of the above-described formation. Similar formations are visualized in S5.6 with dimensions 12x13 mm and 15x22 mm. Conclusion: MRI picture of a hypervascular formations in the liver (HCC, LI-RADS 5).
Planned: Percutaneous radiofrequency ablation (RFA) of S5 lesion, laparoscopic liver segmentectomy S5,6
Operating team: Efanov M.G., Kulikova N.D., Melekhina O.V. (RFA)
